Couple Informational Questionnaire Adobe format (Downloadable)

This form provides me with basic information about the couple that will be needed to customize vows and provides me with a little background on the couple.

 

Service Agreement

This document will be completed once an actual date is confirmed by the couple. This agreement explains my commitments and responsibilities to officiate your ceremony and verifies the quoted fee for service.

 


 

Fee Structure

 

Varies based on travel, time, location and rehearsal. Please contact me for actual fees related to your ceremony request I would be happy to discuss this with you.

 

If you are on active military duty in any branch of the United States Armed Forces and planning on a ceremony in New Hampshire please notify us so that we may provide you with special pricing to thank you for your service to our country.

Cash Payments

Please advise if you will be paying with cash so that we may prepare a receipt for your payment.

 

Personal Checks (please read check policy below)

 

If you will be paying by personal check your ceremony date (s) will be reserved.  Please keep in mind that when using personal checks the balance due must be paid at least 14 days prior to the ceremony date (s) allowing time for the payment to clear.

*Fee for services are negotiable based upon actual travel and additional services or needs and may be packaged together in an effort to reduce costs. A person or couples inability to pay will be considered and we ask that you please contact us to discuss. Once an actual price for services has been provided to a couple and the deposit is paid the service fee and ceremony date (s) will be blocked.

 

*Fee for services includes filing the original certificate of marriage or civil union with the appropriate town/city clerks office once the ceremony is completed. You may request an official copy of your marriage or civil union certificate directly through the appropriate clerks office usually within 3-21 days after the ceremony.

 

*A deposit of 50% of the total will be required to reserve your ceremony date with the remaining balance payable at least 7 days prior to the date of the ceremony unless paying by check. If paying by check the balance must be paid 14 days prior to the ceremony date.

 

 

*The license for the ceremony is an important document. If the Justice of the Peace does not have this document prior to the actual ceremony the Justice of the Peace  will be unable to officiate the ceremony.
